Work Permits

If you are a student currently enrolled in White Plains High School and are between the ages of 14-17 years old, please complete the Work Permit Application below which consists of two forms.  Please submit both completed forms to your designated Guidance House Office. 

Link to form 1- Only Section 1 and Section 2 need to be completed by student and parent  Work Permit Application 

Link to form 2- Physical Fitness Certification form to be completed by school nurse or primary doctor  Work Permit Application - Physical Fitness Cert. 

If you are in need of a duplicate work permit, please contact your Guidance House Office.
